for Pension. ( with residential address / Telephone number)

Form -1 or Form-1(A) Application
for commutation, along with part III, duly approved by the Head of the office.

4.
Two slips bearing the left hand thumb impression duly
attested.
5.
Three copies of passport size joint photograph duly
attested by the head of the office.

Descriptive particulars – Height & Personal marks of
identification
7.
Advance Stamped receipt
8.
Form A - Nomination for payment of arrears of pension
9.
Form for Nomination for payment of commutation of pension
