1. Death certificate

2. Two Specimen signature slips duly attested

3. Two slips bearing the left hand thumb impression of the claimant / guardian duly attested.

4. Three copies of passport size photograph of the claimant / guardian in the case of minor duly attested

5. Advance Stamped receipt.

6. Form-5 Application for Pension. ( with residential address / Telephone number)

7. Legal heir ship Certificate / Succession Certificate if necessary
